The MGB Commission has created three surveys for different segments of the PC(USA). The individual survey asks about interactions between congregations and Presbyteries and supposedly takes about 20-25 minutes to complete, but if you can spare that time for the good of the church, here are the links: individual, session, Presbytery/Synod leader.
